You can request a reimbursement from the mutual funds in your health savings account by submitting a Request for Reimbursement Form to Health Savings Administrators. Send to us at this address:

Reimbursements are withdrawn directly from your Mutual Fund and the reimbursement check will be made payable to you.
Please note: You do not need to send your receipts. Keep them for your records in the event of an IRS audit.
